Renew Health Plan Policy & Terms

By enrolling in a Renew Health membership plan, you agree to the following terms:

Medical Services

Renew Health provides physician-led weight management services through telehealth. Enrollment in a plan does not guarantee treatment, prescription approval, or eligibility for any medication. All treatment decisions are made at the sole discretion of the physician.

Patient Information

You agree to provide accurate, complete, and up-to-date medical information. Providing false or misleading information may result in denial of treatment or termination of services.

HIPAA & Privacy

I acknowledge that I have reviewed or have access to Renew Health's Privacy Policy and Notice of Privacy Practices. I authorize Renew Health to collect, use, store, and disclose my Protected Health Information (PHI) as necessary for treatment, payment, healthcare operations, prescription management, and telehealth services in accordance with applicable privacy laws, including HIPAA.

Telehealth Consent

I consent to receiving healthcare services through telemedicine technologies. I understand that telehealth has limitations compared to in-person care and that certain medical conditions may require referral for in-person evaluation or treatment.

Prescription Policy

Prescription medications, including GLP-1 medications such as semaglutide and tirzepatide, are prescribed only when medically appropriate. Payment for services does not guarantee that a prescription will be issued.

Results Disclaimer

Individual results vary. Renew Health does not guarantee any specific amount of weight loss or health outcome.

Membership Billing

Monthly membership fees are billed according to the selected plan. Unless otherwise stated, monthly membership fees are automatically recurring until canceled.

Cancellation Policy

Memberships may be canceled at any time before the next billing cycle. Cancellation prevents future charges but does not provide a refund for services already rendered or current billing periods.

Refund Policy

Medical consultations, physician reviews, treatment plans, and prescription services are non-refundable once completed. Prescription medications are non-refundable once prescribed, processed, dispensed, or shipped.

Emergency Medical Care

Renew Health does not provide emergency medical services. If you are experiencing a medical emergency, call 911 or seek immediate emergency medical attention.

By enrolling in a Renew Health plan, I acknowledge that I have read, understood, and agree to these Terms and Conditions.
